Very rare document issued by Nazi occupation authorities in Poland in the Generalgouvernement of Hans Frank, in both German and Polish languages that Polish citizens must sign to swear they weren't Jews, neither their ancestor were. If you didn't comply any of the guidelines of the document and you hid your Jewish condition, you would be severely punished.
