Very interesting form to be filled by Polish citizens for a policial record in this case filled in by a Jew!! Documents like these were compulsory so that Nazi authorities had all the Polish population completely controlled and tracked down. And this was also a very reliable method to control the jewish population with the aim to ease the labour of deporting all the jews when they were sent to concentration camps. This document is in very good condition and it's written both in German and Polish. A must for Holocaust related stuff collectors!
