Nice document with pictures of a young Czech guy from Protektorat Böhmen u. Mähren who filled in this form to ask for the issuing of a new Kennkarte or identity card. Document is complete with some water damage, but overall condition is good and it's readable and bilingual, both in German and Czech, which makes it really interesting.
